Children's Health Home
Navigating your child's healthcare can feel overwhelming. Our Children's Health Home program simplifies everything by connecting your family with a dedicated care manager who coordinates appointments, specialists, and community resources — so you can focus entirely on your child.
We work closely with parents to create an all-encompassing care plan supporting both child and family. Through Medicaid funding, this program is available at no cost to eligible families.
HCBS Services
Our HCBS program supports children who are medically fragile or at risk due to mental health challenges. Through Medicaid funding, we provide care that keeps children in the comfort of their homes and communities — promoting independence and avoiding restrictive settings like long-term care facilities.
Services are aligned with each child's cultural and developmental needs, helping families navigate challenges and build a thriving future.
You're Not Alone
Our Peer Support program connects children and families with individuals who have lived experience navigating similar challenges. These real connections inspire hope, build confidence, and create a lasting community of understanding around every family we serve.
Research shows peer support significantly reduces isolation and improves mental health outcomes. When children see others like them thriving — and when parents know they are not alone — it creates a powerful belief that change is possible.
What Is ABA?
Applied Behavior Analysis (ABA) is a science-based therapy that helps children build meaningful life skills through structured learning and positive reinforcement. Backed by decades of research, it's one of the most effective treatments for children with autism — guiding them toward independence, communication, and stronger relationships.
