Children's Health Home

Navigating your child's healthcare can feel overwhelming. Our Children's Health Home program simplifies everything by connecting your family with a dedicated care manager who coordinates appointments, specialists, and community resources — so you can focus entirely on your child.

We work closely with parents to create an all-encompassing care plan supporting both child and family. Through Medicaid funding, this program is available at no cost to eligible families.
